I deeply apologise for missing our appointment on Monday at 11:00 am without informing you. It was highly unprofessional on my part that without explaining my inability to attend the meeting, I didn’t turn up at the scheduled time.

I had no intentions to skip such an important meeting but (reason) one of my immediate family members was hospitalized and therefore I had to attend him. I would like you to know that otherwise I am a punctual person and always adhere to deadlines, but on this particular day, due to such uncertain events, all my plans went for a toss. The irony is that I had been preparing for this meeting for the past one month and was eagerly looking forward to it.

I hope that you will give this important meeting a second thought and consider re-scheduling it. I will give you a call this week and hopefully we can reschedule it.

I would like to thank you in advance for understanding my circumstances and once again, I offer my apologies for not attending the meeting.

    At the outset, I am extremely thankful to you for your formal offer of appointment for the post of Senior executive, to join your esteemed institution on 26th Mar’14 at 09:00 A.M.

    I most regretfully bring to your kind notice that due to unforeseen personal reasons, I couldn’t report to the office at the designated hours on the scheduled date of appointment. Factually, on that ill-fated date, I was informed of the sudden illness of my ailing father, resulting in hospitalization that necessitated my personal attendance.

    I immediately had to rush to my native place and attend to him and ensure that he received the best of medical attention. Literally, I was completely pre-occupied with his treatment, well being and timely recovery. I altogether was rendered helpless and could not either report in person or even formally send my apology for my inability to report to formally accept your offer of appointment.

    I am extremely sorry for not responding on time, considering my awful personal woes and reiterate my total inability to courteously respond in writing. I shall be immensely obliged, if you could condone my inability to join and give me a fresh offer of appointment to your convenience.

    I assure you of my best of efforts if once given a fresh offer, with an assurance to unconditionally accept and do justice to your utmost satisfaction.

    Please accept my deepest apology for missing our appointment on Friday. Of all this year's meetings, this was the one I wanted and needed to attend. Based on our earlier discussion I have made all the reports pertaining to the logistics strategies. But due to unforeseen circumstances I got delayed and could not make it to the meeting.

    However all I can do is offer my sincere apologies, I know it was my obligation to attend the meeting as I had committed to it. But due to a power outage in our area I was delayed. The power outage lasted 4 hours making my entire schedule go haywire. I realize how busy you are and that your time is valuable, but I hope this will not prevent us from rescheduling our appointment. Can we have the meeting on Wednesday at 4pm? If not we can meet anytime according to your schedule. Do let me know. Meeting with you means a great deal to me. I sincerely appreciate your patience and consideration.
