Sample disciplinary letter for misconduct, bad attitude, disrespect, poor performance, absenteeism

We would like to bring to your attention that management has received many complains about your tardiness and prolonged absence from work,

You are well aware that office hours begin at 8.30 am, and as an experienced, you should set an example for the members of our staff. Please keep in mind that as an employee of this company, it is your obligation to inform the office if you are going to be absent, late or going somewhere else before coming into work.

As for your absences without leave, the office must be advised should you be unable to make it to work as you have responsibilities to take care of. At times, your duties can be designated to someone else for the time being, but it would be good for the people at the office to know in advance so as to avoid unnecessary worry.

In lieu of this, your duties as an employee also entail you to keep your communication lines open should someone need to contact you during the day.

Tardiness or absence, along with the difficulty we have communicating with you, may cause certain problems especially in cases of emergency, which as you know, in our line of work is always the case. Please be more aware of the time.

We hope that this letter will act as warning in advance to avoid complications in future.

HR department maintains leave records in its online system and reviews the same periodically. It has been observed that you have taken 52 days leave in the past 4 months.

The leave to your account is fully exhausted. The last 22 days leave has been rendered without payment. You have been notified in the past about excess leaves taken by you in December and you have been warned of strict action

The sales work in your department has been affected seriously and we have received complaints from your sales manager also in this regard. In spite of repeated request, your attendance records have not improved

In view of the above, you are dismissed from services with immediate effect.
